JOB SUMMARY/ESSENTIAL JOB FUNCTIONS:  The Intervention/Outreach Specialist assists youth and families to connect with supportive services through screening and referrals in the Center for Youth Advocacy and Well-Being (CYAW). This position assists children and families in coordinating the delivery of health, wellness, and mental health services by assisting with the navigation of child-serving, educational and human/social services systems.

  DUTIES AND RESPONSIBILITIES :

Interviews youth and families to determine options for securing and maintaining resources to support needs and aids in the navigation of community-based services.
