Job summary
To lead the delivery of the 0-19 Healthy Child Programme, and Public Health Nursing Programme for children and young people with identified targeted health needs in line with the local and national health priorities under the management of one identified HFT Specialist Public Health Practitioner - Health Visitor.

Main duties of the job
This role cements seamless, safe systems operated within the HFT service ensuring targeted CYP health needs are addressed in an efficient, effective way with strong partner service relationships forged within robust communication practices.

Caseload holder responsible for case management, assessment, triage, planning, care delivery using evidenced based interventions for CYP identified as looked after, under support of YJ service requiring HFT intervention.
