Description - External The BMSC Adult Community Clinical Services (ACCS) Program is seeking a dynamic candidate who will serve as a Licensed Practical Nurse on one our integrated teams. The LPN II will provide supports & services to adults with major mental illness in Department of Mental Health ACCS Group Living Environment residential homes as well as in the community. The LPN IIwill provide face-to-face community-based services to assigned persons in their homes, homeless shelters, in hospital settings or any other setting in which the person served resides or currently requires intervention (including in one of the BMSC ACCS 3 24/7 group residential environments).The LPN II will provide behavioral and physical health monitoring and support as well as medication administration and education as needed.
