Responsibilities

• Assist one of the existing three LTCI practitioners with the resident caseload.
• Provide regular proactive visits and conduct on‑site medication reviews.
• Attend care conferences, participate in 24/7 on‑call, and complete documentation in the charts.
• Collaborate with an interdisciplinary care team (physicians, nurse practitioners, nurses, allied health, and care home leadership) to deliver holistic, resident‑centered care.
