**Hospice Homecare LPN -** **Per Diem** **Saratoga&Washington Counties**

**WEEKDAYS 8am to 4pm**

The Community Hospice has a wonderful opportunity for a Per Diem LPN to work as a member of our Hospice Home Care Team caring for patients and families in both the homecare and nursing home settings. Primary region to cover is Saratoga and Washington Counties.

Needs would be on the **WEEKDAYS, no weekends**
