Position Overview


Duties


  • Manage a caseload of 20-23 patients, conducting initial assessments within 24 hours.

  • Coordinate discharge plans and all post-acute care needs.

  • Work closely with the medical team, insurance case managers, and Providence UM case managers to ensure safe and efficient patient discharges.

  • Facilitate discharge and contingency planning, including crisis intervention.

  • Assist patients with navigating the Department of Children and Families (DCF) process.

  • Coordinate services for home care and equipment following discharge.

  • Document pertinent information to facilitate care coordination.

  • Educate patients and caregivers to facilitate discharge and reinforce individualized education.

  • Proficiency in Epic charting system.
