**JOB SUMMARY:**

Penn State Health has an opening for a Nurse Practitioner for Post Acute Care. As part of a multidisciplinary team, patient care will be provided at Long-term care facilities and Rehab Facilities, in conjunction with a Physician Board certified in both Internal Medicine and Geriatric Medicine.
