Description

:Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention. They may care for patients ranging in age from newborn to elderly as noted in age-related category and area of expertise.

Education Qualifications:

This position will assist our patients by utilizing a strong team-based approach to care, with a high integration of APP and MDs for the clinic, and inpatient and dialysis units for all 3 phases of nephrology care.

· Desired skill set: Experience in complex chronic disease management

· Typical Shift: up to FTE per pay period

· 5% clinic work

· 5% hospital work

· 90% dialysis rounding
