UPMC Central PA is seeking an experienced Heart Failure Nurse Navigator to support patients across inpatient and outpatient settings within our nationally recognized Heart Failure Program.

Schedule

+ Monday - Friday, 7:30 AM - 4:00 PM
+ No weekends or UPMC holidays

Location

+ Harrisburg Hospital, Community General (Bloom), and Mechanicsburg, 2020 Technology Parkway
+ Potential support at Carlisle, Hanover, and Memorial sites
+ One site per day
