Position Summary/ Department Summary: Nurse Practitioner, Asthma Community Program
Key Responsibilities: - Identifying patients with high-risk asthma through daily review of hospital admissions for asthma exacerbations, ED logs, chart reviews and referrals from primary care providers, asthma specialists, social workers & community partners for asthma home visits
- Working with the entire care team to determine care needs of a given patient based on his/her clinical condition & identify/steps to remove barriers and improve care
- Training and supporting Community Health Workers in conducting family-centered asthma home visits that include asthma education; assessment of patient's asthma control, medication understanding and adherence, and social needs; home environmental assessments, provision of supplies for asthma trigger reduction & case management
