About the Role

As a Nurse Practitioner you will work independently and collaboratively within an interdisciplinary model to provide advanced clinical expertise and leadership in the provision of health care to patients. This role will provide patient‑focused care and coordination that will optimise the patient journey and improve health outcomes.

About the Team/Service/Location

The Community Mental Health service provides community‑based assessment and treatment services to people, including those presenting with alcohol and drug issues, through both community follow‑up and outpatient clinics. This service also provides crisis response for mental health, dual diagnosis and clinical rehabilitation.
