About the role
We have an exciting opportunity for an
innovative Mātanga Tapuhi Nurse Practitioner (NP) Health of Older People with a
passion for Community based practice.
This
cross-sector role based at
Hutt Hospital improves access to comprehensive gerontology assessment (CGA) and coordinated interventions to support older adults to live safely
in their own homes. To improve equity, the Mātanga Tapuhi NP will work with Māori and Pacific people aged 55 + years who
