Location: Burbank, CA and Glendale, CA
Position Overview:
We are looking for a dedicated and experienced Nurse Practitioner to join our clinical team in Burbank and Glendale, CA. In this role, you will provide in-home health care services, conduct assessments, and deliver compassionate patient care within your assigned territory.
Key Responsibilities

• Home Visits: Conduct 5-7 home visits daily for patients with complex care needs.
• Health Assessments: Perform comprehensive health evaluations, including:
  • Detailed medical history and physical examinations
  • Medication reviews and safety assessments
  • Evaluation of home safety and psychosocial needs
• Documentation: Accurately document all pertinent patient information, including medications, end-of-life preferences, and factors contributing to preventable hospital admissions on the same day as visits.
