Providence Medical Group is seeking a full-time nurse practitioner to join Providence Connections Palliative Care in Portland, Oregon. Connections Palliative Care is part of a growing integrated Home and Community Services program which collaborates with Hospice, Home Health, Geriatrics as well as hospital-based practitioners to care for patients and their families. Join an established interdisciplinary team providing a single, coordinated program for management of chronic conditions or life-limiting illness to patients and families throughout the Pacific Northwest.
* Full-time position, Monday to Friday
* Outpatient care setting, community-based settings, and home visits and occasional hospital setting
* Collaborate with physicians, nurse care manages, social workers, pharmacists, behaviorists, chaplains, and other team members
