Position Overview

            1. Job Descriptions  Attend to general health and care of all residents. 
Assist the Social Worker in performing Resident Assessment Form (RAF) for potential residents into the HOME. 
Update colleagues on residents’ medical and nursing matters. 
Advise Head of Home and Deputy Head of Home on admission of potential residents and discharge of residents from medical and nursing viewpoints. 
To conduct RAF for residents every six months or upon request from Head of Home. 
Accompany residents to clinics and hospitals. 
To update colleagues of residents’ upcoming or rescheduled medical appointments. 
Required to be on “standby” for emergency and crisis of residents. 
To conduct and document weekly hypo count tests for residents. 
Update residents’ medical records regularly. 
Top up residents’ medication when necessary, and to prepare medication when residents go on “home leave.”
