**Description:**
To provide Occupational Therapy services to patients to include evaluation, assessment, treatment program planning and implementation, and related documentation, and communication.
**ESSENTIAL FUNCTIONS** **:**

**Evaluates patients to obtain and interpret data necessary for treatment planning and implementation.**

**Develops treatment plans, directs, supervises and treats patients in accordance with treatment plan using treatment modalities.**

**Prepares written report of diagnostic findings, intervention techniques used, progress made and follow-up status in a time frame according to policy and procedure.**

**Consults and communicates with patient, family and other multi disciplinary team members and contracted agencies.**

**Assist multidisciplinary team with discharge planning and established home therapy programs.**
