Responsibilities

• Provide comprehensive medication management including reconciliation, dose optimization, and adherence support


• Partner with physicians and interdisciplinary teams to order individualized care plans under a CPA


• Monitor therapy effectiveness and safety to identify optimization opportunities


• Support quality-improvement and REMS initiatives


• Assist operations with complex prior authorizations and workflow improvements

Requirements

• Doctor of Pharmacy (PharmD)


• Licensed pharmacist in California


• PGY2 Oncology Residency completion OR PGY1 Ambulatory Care Residency with 3 years oncology experience OR 3 years oncology clinical pharmacy experience including 1 year under a CPA


• Proficiency in Microsoft Office tools and web-based platforms
