**Job Description**

The Outreach Program Coordinator is responsible for assisting in the development and dissemination of inclusive promotional and educational materials tailored to the needs of the grant's target communities, East Harlem and the South Bronx. They will also provide direct, empathetic guidance to patients seeking care at the Long COVID clinic and work collaboratively with the clinical care team. Build and maintain trusted relationships with community members, community-based organizations (CBOs), and the Community Advisory Board.

**Qualifications**

+ Bachelor’s degree in Public Health, Social Sciences etc. or equivalent combination of education and experience.
+ One (1) year work-related experience.

**Responsibilities**

+ Coordinates with Long COVID clinician(s), other providers and social work, as needed.
