Position Summary:

As a Patient Care Coordinators, we are responsible for maintaining patient information and coordinating with caregivers and insurance companies. We handle all requests and inquiries for patient information and disperse this information according to federal and state regulations. Ensuring compliance with the medical facility’s policies and procedures and developing interdisciplinary care plans for patients is part of our main responsibilities.

Responsibilities

• Assists callers with appointment scheduling, obtaining patient demographics, insurance and financial information.
• Process incoming referrals, verifying benefits, eligibility and authorization for both new and established patients.
• Coordinate and respond to patients' and referring providers’ concerns in a timely manner and ensure patient satisfaction.
