Mindify Wellness and Care is seeking an outgoing, compassionate, and relationship-driven Patient Care Coordinator to serve as a trusted presence to represent Mindify within adult day care centers throughout New Jersey.

This individual will educate participants about our behavioral health services, build meaningful relationships with adult day care center attendees, support patient engagement and continuity of care, asist with translating during therapy and/or psych sessions, and help connect individuals with Mindify mental health services.

This role is ideal for someone who enjoys working directly with older adults, educating and supporting patients, building lasting relationships, and making a meaningful difference in the lives of others.
