Our client in Putnam, Connecticut is seeking a Patient Care Coordinator for a contract opportunity. This role is ideal for someone who is highly organized, compassionate, and experienced in supporting patients through scheduling, communication, and administrative coordination in a healthcare setting.

Key Responsibilities:

+ Coordinate patient appointments, follow-ups, and referrals

+ Serve as a primary point of contact for patients, providers, and internal staff

+ Verify and update patient information in the system

+ Assist with insurance verification, authorizations, and related documentation

+ Handle inbound calls and respond to patient questions with professionalism and empathy

+ Maintain accurate records and ensure timely documentation
