Patient Service Officer (Primary Care Coordinator) The Primary Care Coordinator supports the implementation and administration of Primary Care Network (PCN) and Healthier SG (HSG) programmes. The role is responsible for programme reporting, compliance monitoring, service coordination, and operational support to ensure participating clinics meet programme requirements and performance targets.
Key accountabilities - Prepare, collate, and validate programme data for reporting, submissions, and compliance tracking.
- Prepare administrative reports required by MOH/AIC for programme funding and performance reporting.
- Coordinate PCN ancillary services such as Diabetic Retinal Photography (DRP), Diabetic Foot Screening (DFS), Nurse Counselling, and Spirometry.
