Are you a compassionate and motivated Registered Nurse looking to develop your career within primary care? South Sefton Primary Care Network (PCN) is seeking a Nurse to join our growing multidisciplinary team and play a vital role in delivering high-quality, proactive care to some of our most vulnerable patient populations.

This is an exciting opportunity to work across a network of practices, providing personalised care for housebound patients with long-term conditions and delivering Learning Disability Annual Health Checks that help reduce health inequalities and improve patient outcomes. You will work closely with GPs, Advanced Nurse Practitioners, Care Coordinators & other System Partners to support patients in maintaining their health and independence within their own homes and communities.
