**EDUCATION:**

+ Graduate of an accredited physical therapy program
+ OR graduate of an accredited physical therapy program that can be demonstrated with a valid PT license

**CERTIFICATION & LICENSURE:**

+ OR Will also consider approved license applicant or resident training program employee.
