**Job Summary and Responsibilities**

**Position Summary:**

The Clinical Pharmacist is responsible for development, implementation, measuring outcome and education of the antimicrobial stewardship program for the facility, analyzing and recommending action plan(s) for reported medication errors/adverse drug reactions, medication formulary management, order sets initiation/updates, evaluate clinical programs, preparation of clinical protocols/investigational studies, and other clinical activities requested by medical staff and pharmacy leadership. This will be accomplished through collaboration, coordination and communication with the Pharmacy Administration, Infection Preventionists, Pharmacists, Medical Staff and other healthcare workers.
