Summary:

The Pharmacy Medication History Technician (MHT) is responsible for obtaining and documenting patients’ current prior to admission medication(s) into the hospital’s electronic medical record, ensuring the information is accurate and up to date for use by providers, nurses, and pharmacists during the patients’ admission, discharge, and transition of care.

Responsibilities:

1. Interviews patients, family members, guardians, etc. to obtain complete medication histories.

2. Communicates with providers, retail pharmacies, living facilities, etc. for additional medication history information and verify accuracy.

3. Document medication histories in the patient’s electronic medical record.
