This is an excellent opportunity for an experienced Physical Therapist to provide skilled home health services, initiate Plans of Care (POC) and OASIS assessments, collaborate with interdisciplinary care teams, supervise Physical Therapist Assistants (PTAs) in accordance with state regulations, and deliver high-quality patient-centered care throughout the assigned coverage area.
