We have an exciting opportunity to join our team as a Physical Therapist - PV, Home Health Care, Nassau/Suffolk locations.

In this role, the successful candidate provides safe, therapeutic age appropriate physical therapy to promote the improvement of functional mobility, and physical ability, and quality of life. Uses clinical expertise to plan the delivery of care through examination, evaluation, planning, implementation and re-assessment.

Job Responsibilities:

Educates patient and family regarding patient s functional status and plan of care. Provides a home exercise program and instruct on home safety factors.

Re-assesses patient progress toward short termlong term goals and modifies the plan of care accordingly.

Completes and maintains accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education, intervention and evaluation according to Federal, Department of...
