**Position Title:** Physician, Post-Acute Long-Term Care

**Position Duties:** Monitor, examine and provide follow-up treatment to patients, primarily the elderly, who are being discharged to skilled care or nursing facilities for short-term rehabilitation after treatment for acute conditions. Provide specialized palliative care and treatment to patients who require pain management or other palliative care as part of their follow-up care and treatment. Coordinate follow-up care with physician specialists at medical centers who treated the acute condition.

**Position Qualifications** **:** Doctor of Medicine or Doctor of Osteopathy degree required. Foreign equivalent degree accepted. Must have a total of 60 months of training as follows: (1) 36 months of training in a residency in either internal medicine or family medicine; (2) 12 months of training in a fellowship in geriatric medicine; and (3) 12 months of training in a fellowship in palliative medicine. **Must...
