We are seeking two new practical nurses for Oma Häme's home care assessment and rehabilitation team for elderly care, on fixed-term contracts from August 3, 2026, to March 30, 2027, and from August 3, 2026, to April 29, 2027.

The tasks are based in the Hämeenlinna region team, which covers the central city of Hämeenlinna, Rengo, Kalvola, Janakkala, and Hattula. Our office is located in Hämeenlinna.

Team Purpose

The Home Care Assessment and Rehabilitation Team conducts assessment and rehabilitation periods for clients assessed to need regular home care services. The main task is to clarify, together with the client, their relatives, and other cooperation partners, the client's resources, functional capacity, rehabilitation possibilities, and to actively evaluate the need for services.
