Position Overview



We are seeking a Primary Care Advanced Practice Provider (PA-C/ARNP) who thrives in a value-based, patient-centered care environment. The ideal candidate will work closely with a dedicated interdisciplinary team- including a Physician, nurse care manager, and clinical pharmacist-to deliver high-touch, coordinated care to a defined patient panel.



This is not a typical fee-for-service role. Our providers focus on long-term outcomes rather than visit volume. In this role, you will collaborate with other physicians, nurses, therapists, and pharmacists to deliver comprehensive care to patients enrolled in value-based care programs, such as Medicare Shared Savings Programs (MSSP), and MedAdvantage.
