This facility is seeking a Primary Care Home Health Physician or Nurse Practitioner (NP) for locum tenens support as they look to fill a current need.

Schedule: Flexible schedule based on availability (Part-Time or Full-Time basis)

Job Setting: Home Care

Types of Cases: In-home health and wellness assessments 

Credentialing Timeframe: 6-8 Weeks

Immediate start upon credentialing 

Must have an active Oregon state license
