Coordinate and support the daily operations of assigned programs.

**Job Responsibilities and Requirements:**

PRIMARY RESPONSIBILITIES

+ Prepares schedules to provide service to program clients. Coordinates all new referrals from external and internal sources.
+ Answers the telephone and responds to general inquiries regarding the program. Provides basic education and guidance to clients.
+ Facilitates the development, implementation and evaluation of program services and initiatives.
+ Plans, implements, evaluates, and coordinates appropriate patient care.
