Summary The Community Care Service (CCS) coordinates skilled care to Veterans purchased from outside community vendors. The CCS works collaboratively with local VA Medial Center providers, providers at other VA Medical Centers, and with local community vendors including home care, hospice agencies, local hospitals, long term care facilities, etc. The purpose of the work is to complete specific procedures and processes, which effect accuracy, reliability, and productivity of the Community Care Service. Responsibilities This position relieves the supervisor and clinical staff of various routine and administrative work processes and ensures work complies with appropriate policy and procedure.
