Job Description:

• Oversee and enhance hospital-wide social needs screening and referral workflows, ensuring patients are connected to appropriate community resources while supporting program goals and performance targets.
• Recruit, train, and supervise Health Advocates, coordinate stakeholder engagement across hospital departments and community organizations, and prepare reports, presentations, and data analyses to support program success and continuous improvement.
