Summary of Position: The Qualified Mental Health Professional – Community Services (QMHP-CS) is responsible for providing Targeted Case Management and Rehabilitative Services to youth with serious emotional disturbances and their families.
Services may be provided in the participant’s home or appropriate community setting.QMHP-CS will deliver case management and skills-based interventions targeted increasing the youth’s natural supports and strengths, fostering resilience and recovery. QMHP-CS is responsible for accurately documenting services, attending Wraparound Team meetings as needed, and communicating participant progress to supervisors, team members, and the referring authority.
