Job Description/Requirements.

**Proposed Schedule** : 40 hours per week. Monday-Friday 8am-4:30pm.

_Note: This is a remote position and candidates must reside within reasonable driving distance to Queensbury, NY._

The Referral Specialist acts as a liaison between patients, providers, and health center staff members by providing prompt and professional service for the patient’s referral and follow-up needs.
• Monitors and maintains referral inbox for assigned health center locations
• Verifies patient demographic and insurance information and updates as necessary. Works with Eligibility Team to verify patient health insurance eligibility as needed
• Completes the electronic Referral Order process for patients needing internal and/or external referrals for health care services
