Summary The Cincinnati VA Medical Center is seeking a Community Care Coordinator Registered Nurse. The RN will demonstrate the integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients. The RN will assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
