POSITION SUMMARY
The Nurse Care Manager utilizes expert clinical skills and a disease specific knowledge base to assist identified patients through the continuum of care in an effort to provide appropriate, quality care in a cost-effective manner. The Nurse Care Manager conducts daily assessments to identify high risk patients and manages their care during the acute care episode. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.
QUALIFICATIONS
EDUCATION: Graduate of an accredited school of nursing required. Bachelor's in Nursing preferred.
LICENSE/CERTIFICATION: Current licensure as a Registered Nurse in the State of New Jersey and BLSHCP Certification required 6 months from date of hire or transfer required. For some specialties, a Master's degree in Nursing may be required, with an...
