Location: Honesdale, PA Schedule: 8 AM – 5 PM, Monday–Friday | 6 units daily / 30 units weekly On‑Call: 1 weekend per month + 1 evening per week Territory: Honesdale and approximately 15–25 miles surrounding the community Travel: Local travel across multiple counties required

You'll work independently in the field while maintaining strong communication with interdisciplinary partners to ensure patients receive dignified, high‑quality end‑of‑life care.
