Under the general direction of the Manager, Renal Services and the Nurse Clinician responsible for training and follow‑up of Peritoneal Dialysis / Home Hemodialysis patients, ensuring safe patient and family practice in the home, and interfacing with outside agencies and hospitals on supplies and equipment delivery for Peritoneal and Home Hemodialysis patients.
