We are looking for experienced USRNs to join a healthcare team as a Care Coordinator.

𝗟𝗼𝗰𝗮𝘁𝗶𝗼𝗻: 𝗔𝗹𝗮𝗯𝗮𝗻𝗴, 𝗠𝘂𝗻𝘁𝗶𝗻𝗹𝘂𝗽𝗮 𝗖𝗶𝘁𝘆

𝗪𝗼𝗿𝗸 𝗦𝗲𝘁𝘂𝗽: 𝗢𝗻𝘀𝗶𝘁𝗲

𝗥𝗲𝗾𝘂𝗶𝗿𝗲𝗺𝗲𝗻𝘁𝘀:

• Active USRN License.

• 6 months to 5 years of clinical experience.

• Experience in acute care, ambulatory care, home health, care management, or case management.

• Strong patient assessment and care coordination skills.

• Excellent communication and documentation abilities.

𝗥𝗲𝘀𝗽𝗼𝗻𝘀𝗶𝗯𝗶𝗹𝗶𝘁𝗶𝗲𝘀:

• Conduct telephonic and virtual patient assessments.

• Develop and manage individualized care plans.

• Coordinate care across providers and healthcare services.

• Educate patients and families on treatment plans and self-management strategies.

• Monitor patient progress and escalate clinical concerns when necessary.
