Schedule: All shifts available
Pay Range: $40 per hour (based on license and experience)

As a Registered Nurse on the Pediatric Transition Team, you’ll ensure children and families experience a safe, supported, and compassionate transition from hospital to home. Your expertise helps prevent readmissions, builds family confidence, and makes an immediate impact where it matters most. In this role, you will:

Assess patients’ post-acute needs and coordinate care with families, discharge providers, and our interdisciplinary team. 

Deliver hands-on pediatric nursing in the home, including medications, treatments, and therapies. 

Educate and support families to strengthen outcomes and ease the transition from hospital to home.
