The post holder will work within the scope of practice, putting the health, wellbeing and safety of their patients first as they assess, plan, implement, and evaluate their needs.

- Provide and coordinate patient care.

- Assess and identify patient needs, implement and monitor the patient’s medical plan and treatment.
- Work with a team of physicians and other health care specialists or allied health professionals in various settings.
- Perform systematic patient assessment, re‑assessment and collect pertinent data about the patient’s health or situation.
