Job Description

Responsibilities:
 
You will be working with a multidisciplinary community team to provide medical oversight, recommendations and treatment to elderly patients with mental illness and dementia residing in the community. You will need to be well-versed with the patients’ psychiatric conditions and dementia.
 
You are required to conduct home visits and assess patients in their home setting; which could also be in nursing home or care centres in all areas within Singapore.
 
You will need to work with various community agencies to co-manage patients. Your job includes educating the patients, caregivers and community partners. In addition, you are also required to conduct training and workshop to partner agencies.
 
You are also responsible for maintaining the standards for professional medical practice and work alongside the rest of the clinical team within IMH and NHG.
