What you’ll do:
Assess patients' physical and psychosocial status, intervening preemptively to manage potential crises. And provide patient-centered care with sensitivity to cultural and developmental needs. Collaborate and communicate with interdisciplinary teams for discharge planning and holistic patient care. And facilitate patient education and support the transition of care through the continuum.
