We are seeking an exceptionally skilled RN Case Manager to join our dedicated Case Management team. Responsibilities
+ Perform evaluations and assessments within the established timeframes
+ Detail appropriate reviews for assigned patients using our utilization review tool
+ Provide telephonic reviews for identified contracted/private patients and collaborate with on-site and/or outside reviewers
+ Keep patients informed of progress and provide information related to disease progression
+ Collaborate with discharge planners to make orders and arrange for home care equipment and healthcare needs
