Territory Clinical Manager Roswell

Schedule: M-F 8AM-5PM in Office

Responsibilities:

·Supervise day-to-day clinical and office operations; remain available to staff and patients during business hours.

·Ensure all services meet professional standards, agency policies/procedures, and state/federal regulatory requirements.

·Coordinate patient referrals and care with the interdisciplinary team.

·Oversee the development, implementation, and updating of individualized plans of care.

·Evaluate the quality of clinical care and continually assess patient needs.

·Collaborate with clinicians and physicians on patient transfers, discharges, or changes in condition.

·Manage OASIS documentation, edits, discharge processes, and related data.

·Monitor key performance indicators (KPIs), workflows, and escalations.
