_ContinuCare Pathway_ is a pharmacy-to-supplier referral program created by Cardinal Health to help retail pharmacies seamlessly refer patients to Cardinal Health at-Home Solutions. It simplifies the fulfillment and direct-to-patient home delivery of diabetes supplies (such as continuous glucose monitors) and navigates complex insurance requirements. The program is designed to reduce the administrative burden on retail pharmacists.
This Director will lead national business development efforts focused on prospecting and engaging targeted independent pharmacies and buying groups to build awareness and adoption of the ContinuCare Pathway CGM & BGM solution. The key priority of this Director is to position the organization as a trusted partner in diabetes care solutions.
