About the role

Senior Care Assistant at Scholars Mews Care Home in Stratford‑upon‑Avon will support and work alongside the care team to deliver high standards of personal care, providing a safe, caring, innovative, open, supportive and inclusive environment for residents, visitors and staff.

Key responsibilities

• Monitoring the wellbeing of each resident, being aware of any changes affecting care plans and updating records accordingly.
• Participating in the person‑centred care planning process, implementing, monitoring and evaluating monthly plans.
• Supporting residents with their medication requirements.
• Undertaking staff supervisions and supporting staff development where required.
• Acting as the key point of contact for residents, their relatives and visitors, and coordinating with health and social care professionals.
